Hi There,
Ensure that your MS Form has fields to capture the employee's name and the date. Then, when configuring the Power Automate flow make sure after the "When a new response is submitted" trigger, add the "Get response details" action for MS Forms and use the Response Id from the trigger. In the action where you're creating the PDF or forming the content for the email, reference the employee's name and date fields using dynamic content. They should appear as outputs from the "Get response details" action.
Verify that these fields in MS Form are mandatory, ensuring they are always filled. If you're using a third-party tool or a custom connector to generate the PDF, ensure that it is correctly mapping the employee's name and date fields from the form response.
By following these steps, the employee's name and date should populate in the triggered email with the PDF. Always test your flow to ensure proper functionality.
Please give kudos and mark as solution if it helps.
Thanks,
