Hello team

I created a flow to send an approval to HR but I need feedback from HR when they approved the request

Is any way that I can add questions to the approval email besides the comments box?

I would like to add a couple of questions into this form to save them into a Sharepoint and store the date by

@Anonymous - There is no way to customize approval card at this point. But you can configure custom actions and then take a decision based on that. Is it possible in your requirement? See below example
