Looking for Sharepoint form (list) to power automate recommendation & flow features

Hello, I was looking for recommendations or opinions on what templates to use for sharepoint form to power automate. My sharepoint request form (list) consists of questions (in each field) with yes or no checkboxes, fill in multiple lines of texts, add attachments/upload files for people who fill in the form. My form is for the “change freeze.” And my team would like to get notified when the  form gets submitted, but depending on which service they select (for one of the questions), depending on which a respondent clicks, so that it’ll be automated to send to a certain team member and vice versa on if they click on a different option.  Also is there a feature in power automate that allows a respondent to see that the request is being looked at or “in progress” as well as a notification for accepting or rejecting the request form? That’ll notify a respondent/user who has previously submitted the form. I think that this is a “flow feature” I believe is what I’m looking towards as well. Please do let me know if this makes sense or not, I can set up a meeting if needed to explain this as well for what I’m looking for.

It sounds like you might want to use two Flows and also leverage some native SharePoint functionality.

First, You can create a Flow that runs when a new item is created - use the SharePoint When an item is created trigger.  Then, you can use a condition to check to see where to send the email and then an Office 365 Outlook action to send the email.

You can create a second Flow using the SharePoint When an item or file is modified trigger.  Then use a condition and some additional logic to see if you should send an email to the user.

You can create a SharePoint view that will show a user the status of their items.

My suggestion is to give it a try and post any questions that you may have here along with screen shots of your Flow.
