I created a custom form to be the base UI when adding items to a SharePoint list. The list is being viewed through Teams. The form displays normally on some occasions but reverted back to being very tiny.

Please try the following steps
1. Copy the SharePoint list URL. Ex:
https://yousite.sharepoint.com/sites/appcatalog/spfx/Lists/EmpData/AllItems.aspx
2. Go to Teams > Channel > Click the "+" to Add a tab
3. Add the copied SharePoint list URL
Output:
