Append Comments with Attachments

Hi,

I'd like to know how to build a structure for append comments with attachments, it should able to record below items at same post:

1. Who created date and time

2. Comments

3. Attachments (if any)

 

Any suggestion ? thanks!

    @AppsNeed You can create a SharePoint list with a Date and Timecolumn for CreatedBy and a Single line of text column for Comments. Attachments are turned on by default for share point lists.
