Custom list form created using Power App is not loading

we have a custom list form created in Power Apps, which was working fine till last friday(27th September 2019). When i checked on 2nd October, the form is not opening when clicking on the new button. Tried republishing the form, removed and added all data sources etc. still the form is not loading.Attached the image whichs shows the custom column added. I can see version changes in the Power app will it implact the form functionality? Please help me to resolve the issue. Thanks in advance.

Hi @viji_avinesh ,

Do you mean that you can not open sharepoint custom form?

Could you tell me how do you open this form that displayed unsuccessfully?

I've made a similar test and could load custom form successfully.

I chose one item and click "Edit".

I suggest you clean your browser's cache, update the browser to the latest version and reopen your sharepoint list.

Try to refresh your web page.

If the problem still exists, try to restore your form to other version and see whether it could load successfully.

Thank you, not open means, when i click on the 'New' button nothing happnes. List is attached to a content type which is having 40+ controls will that cause any issue? I tried clearing the cache and restoring to old version but no luck?
