Power Platform Community Forum Thread Details

Hello,

I am using power apps to switch out manual tracking that my company has done in SharePoint under list tracking. I have gotten as far as to get all of the typical questions that we would ask into the app and formatted some questions to have drop down boxes for typical situations, however when testing the app and saving a response I get the following errors and can't seem to figure out my way around them. Would anyone be able to help me?
