Creating a working checklist using a form through Sharepoint

Hello, I'm trying to create a checklist that users will fill out, and once they do this data will be submitted into the Sharepoint. 

 

However, I'm getting errors sending this into SharePoint. 

 

I was following a video and was told to put colCategoryData in this field's "default" and "update" formula boxes. However, error saying "error in field, try revising".

but nothing is able to be submitted into the form. 

 

submit button formula on select: SubmitForm(Form2)

checkbox default value: ThisItem.Value

checkbox onCheck: Collect(colCategoryData, ThisItem)

checkbox onUncheck: Remove(colCategoryData, ThisItem)

gallery items formula: Choices([@Checklist_5].Checklist)

 

I don't believe I'm missing any other important info but I'll forward on anything necessary. Thank you!

    What type of choice field (single or multi-select) are you trying to put colCategory in and what is the Update of your data card ?
