Approval on SharePoint Item Creation - not flowing

I have an approval that first loads some information to SharePoint based on some criteria. Then it sends an approval (everything up to this point it working fine)... It is then supposed to take the response and load it back into SharePoint... This seems to be where it fails and never even runs through that loop. Any ideas what I did wrong?

 

This part seems to work ok:

The Approval E-mail comes through fine as well

This part doesn't seem to even be going into the Apply to Each:

    Hi @enswitzer 

     

    It looks correct, so I'm finding it strange that it is failing.

     

    Can you show me a run that is failing with the error message? Probably with that, we can figure this out.

    I think it has something to do with the apply to each,
