The only change was that the password of the account was modified.
I just had the same thing happen to me, I have been using it over 1 year in my app and had no issues. No changes were made that I know of, now the images do not load for anyone using the app. I used the Office 365 User connector to get the Photo V2 as a workaround, however, its taking longer for the app to load. I would like to go back to the ThisItem.'Created By'.Picture as that is clean/simple and requires 1 less connector. Any suggestions on why it would stop working or things to try to get it back? I created another simple app to check the code and it still did not pull up any pictures.

If you have the SharePoint site open in another tab in the same browser, then refresh the app - Do the pictures display now?
I wonder if it was a hiccup in the system. Its working now without changing anything.
