Hello,

This might be a complex one but I cant find a straight answer

To break it down

I have list of employees with dates, on these dates they get sent a automated email with a form attached (This is working fine)
Now I need to find away to attach the answers to that form to the sharepoint list item specific to them, either via their name, id number or email address but I cant seem to work this out in my mind

So basically

Form gets filled out

Scans the sharepoint list for matching information (Either Email or full name)
Attaches the form or part of the form to the item in the list

Anyone have any advice
