App crashes consistently after Submitting form

I have an App that works fine until I add a Form that adds an attachment to an existing record in Sharepoint.

After I have added the Form which only saves the attachment to the Sharepoint record I get an App crash. The attachment saves no problem.

If I never do an add/delete of the attachment the App works fine. e.g. Patching records etc.

The error I get is:

I have no errors in the App itself. Has anyone seen this before?
