Seeing double screens of form on load in SP

Hi All,

I am up against some weird stuff again.

My form, at times, would load to show 2 duplicate screens, does anyone have an idea as to why that would happen.

It's happening 1/50 times, and seems to put my testers off, including me. 

Can't be telling them to keep reloading the page and form
