I created a flow in June and it's not somewhat different if I try to make the exact same one.
For example, it used to be "responses" instead of outcomes and then the approver comments used to be "comments" now its response comments and it does apply to each..Outcomes instead of Response

That is correct. Microsoft rolled out updated versions of the Approval actions recently. A number of things changed including the things that you mentioned. The old version of the approval is no longer available. Old flows should continue to run, but you need to use the new actions to create new flows.
