Form with a Link and need to pull information FROM SharePoint

I have a form that includes a link to a SharePoint list. I need Flow to take the link and get the information from SharePoint and send an email to the person who submitted the form so they can validate the information from SharePoint is accurate and up-to-date.
 
Ideally, a button to confirm could be used. Otherwise, they Form submitter will reply to the email with a "y" or "no". 
 
That condition response above will trigger another flow to create a Planner task - That is NOT part of this ask.
