Help building an automated flow

Hi, entry-level user here. Trying to create a basic flow based on the following criteria and it keeps failing - would welcome any advice. Thanks!

Criteria: when a new email arrives, AND includes [specific email subject], AND includes an attachment

Then: forward the email attachment to a specific email recipient, with a new (custom) email body and a new (custom) email subject

Let me know if the above is not sufficient to provide guidance. Thanks!

Thanks, @Nived_Nambiar There isn't a specific error as far as I can tell, I just receive an email indicating my flow has failed. When I attempt to use the flow checker for my flow, it finds no errors. Attached are a few screenshots, including the email alert I receive about the failed flow, as well as snips of my flow settings. Thanks!
