Getting Error while creating a flow for capturing response from MS forms in Sharpoint

I am getting the below error while i try to create a response flow for capturing the MS forms to sharepoint.

Failed to retrieve dynamic outputs. As a result, this operation's outputs might not be visible in subsequent actions. Error details: The dynamic operation request to API 'microsoftforms' operation 'GetQuestions' failed with status code 'NotFound'. This may indicate invalid input parameters. Error response: { "error": { "code": "674", "message": "CDB TableNotFound. Inner Message: {\"error\":{\"code\":\"NotFound\",\"message\":\"Table 'T7B12QF2DCQRX4LY7MT9CSR2O0' cannot be found\",\"innerError\":{\"code\":\"TableNotFound\",\"params\":[\"T7B12QF2DCQRX4LY7MT9CSR2O0\"]}}}", "@ms.form.error.type": "ExpectedFailure"

When i did research they have suggested we need to adjust the permission for the form as a fix for the above error. 

What are the required permission adjustments that we need to do for the above error to be fixed as the form was not created by some other team.

    If you still need assistance with this, please share an image of the configuration of your workflow. Be sure to expand the details of the action that gets the form response. There is not much anyone can do without more information except to tell you that you probably don't have something configured correctly.

    Also, I don't really know what you mean by, "What are the required permission adjustments that we need to do for the above error to be fixed as the form was not created by some other team." If you are trying to say that you are creating the flow, but you are not the one who created the form, then that is most likely the issue. The owner of the flow also needs to be the owner of the form.

    However, it is confusing since you reference "Microsoft Forms in SharePoint" which does not exist. There are Microsoft Forms (Microsoft Forms - Free tool to create online surveys, forms, polls, and quizzes), and there are SharePoint forms. Which one you are actually using makes a difference in the answer. A screen shot of your flow will clear this up.
