@dawnellie If you are using the New Designer with your flow I would recommend switching to the Classic Designer as the New Designer is still buggy especially when it comes to using the MS Forms Trigger and Action.
If switching to the Classic Designer doesn't help, you may need to rebuild your flow in the Classic Designer. Ensure you toggle off the new designer before making any edits to your new flow.
