    Hi @vividichtarosa

    I think you have used multiple persons for approval for like below

    If yes and if you want to capture the response for all responders in that column. Try this approach.

    Also make sure to make Last Approval column as of multiline type

    I have designed a sample flow for this. see below -

    1. I have used trigger - when item is created so to trigger the process when list item is created.

    2. Use Start and wait for approval action like below.

    Nived_Nambiar_3-1718469620566.png

    3. Now use select action to get required details from responses dynamic content as shown below

    Nived_Nambiar_4-1718470061420.png

    Expression used-

    concat(if(equals(item()?['approverResponse'],'Approve'),'Approved By ','Rejected By '),item()?['responder/displayName'],' on (',formatDateTime(item()?['responseDate'],'yyyy-MM-dd'),')'

    4. Now use update item to update the item's Last Approval column value as below
    Nived_Nambiar_5-1718470127986.png

    Expression -

    join(body('Select'),decodeUriComponent('%0A'))

    Now see - this would how bot/flow updates the column.

    Hope this is what as expected.

    Thanks & Regards,
