One user can't delete attachments in a Form (Dataverse)

We are having an issue where a single user can't delete attachments in a form. When the form is in edit mode, he can click on the "X" button, but upon saving the form, the attachment does not get deleted.

Other users don't have this problem. We've tried clearing the cache on his browser (Chrome), but the problem persists. Any clues what might be causing this behavior?

Have you checked the security roles / permissions for this user and compared to users that don't have issues?

Yes, I've checked the security roles for the entity in question. The user security role allows all access to that entity. Are the attachments however, stored in a different entity?

In any case, I haven't made any changes & the problem only began some two weeks ago (due to no change on our part).

Is there anything else I can troubleshoot?

Just a quick note to say thanks @BCBuizer
It took a while to schedule a debugging session with the user using Monitor, but it showed pretty quick that permissions on the attachment (note) entity were out, so it became a fairly easy fix.
