I'm actually using html code to pass parameters to powerapp to do some filtering, but when I embed my canvas I'm getting "404 Not Found" error upon loading my app in model driven forms.

I'v made a similar test but did not encounted the porblem you mentioned.
I suggest you to check the code of JS part
