Getting an approval notification on the 30th day whether it is approved or rejected ?

Hi Guys,

I'm getting an approval notification on the 30th day whether it is approved or rejected. How can I handle this? I notification should not come after it gets approved...!

Can you guys help me on this..!

Below image is the approval action, which is in "Apply to each"
