Create the form record first and then the update contact.

    I had another thought about your business case you do not need to create the other lookups ..Just the lookup we set over our teams call ..configure the other forms to be like form 1 , This is a better and easier approach and will serve your other issue for generating the PDF since you will have 1 Record.
