Creating new item in SharePoint list, generated new item in other SharePoint list

Hi - 

 

I am multiple list that I need to pull information from - When I create a new item in my Site Survey list, I would like to generate a new Item in my Site Code list - I can get the item created, however the remaining data is not coming over (Address, City, State, Country) 

My Flow:

    I had my list set to lookup columns -- that was the issue, resolved
