Form to populate Word Template

I've created a form with 45 questions which upon submission populates a Word template via Flow. The problem I'm having is that within the Flow the 'Populate Microsoft Word Template' step only list 11 of the questions. I've double checked my original template in word and it matches up to the form. I do know, and not sure if this make a difference, is that some of the questions, including content controls, created in the template were copied and pasted. However, I did make sure I changed each of those content controls to ensure they were different than that was copied. For example, copied content control looks like this [your name], pasted changed to [phone number]. Note: All question from the form are available. I just can't match it up to the Word template. Below is a screen shot. Also note: Some of the questions from the form and template are repeated, but I made sure to identify them differently in each where necessary.
