Approval Flow to add lines to a Share point List from Form.

Hi,

 

I am trying to create a flow that does the following:

 

Form filled in:

Goes to approver.

They accept, a mail is returned to the requestor saying accepted the information is added to the list.

They reject, a mail is returned to the requestor saying the reason for the rejection. 

 

I keep then getting more and more apply to eaches every time.

I've tried setting the output from the previous response to "Responses Approver response". Then in the condition the same. then setting as equal to "approve"

 

like this:

I just keep getting an error.

 

Any thoughts?
