Creating Word Doc & PDF based on MS Forms Response

Hi,

I'm trying to create a form where flow automate will create a word doc based on the new unique responses.

 

Eg if responder keys in a value in Q1, only new unique values will trigger the creation of the word doc.

 

Eg. If responder selected option A in Q2, MS flow will create a word doc based on word doc template A. If responder selected option B in Q2, MS flow will create a word doc based on word doc template B. Both templates are saved in the same folder in One Drive.

 

The word doc will need to get response in Q3, eg Name of Responder and fill into the word doc. (eg, Mr <Name of Responder>)

 

File will need to be then saved in another location in One Drive (in a new folder created with a specific file name) and also emailed to the me and other recipients

    When you have multiple possible values, and want to have different workflows for each. You can use Switch Action.
