Power Platform Community Forum Thread Details

Here is the scenario. I am looking to create a task request library in SharePoint and have a single form for all requests. Some requests might have the exact same details, but might need to be implemented in two separate environments, meaning two separate teams would need to work on it. So, things like Requestor name, Expected Date, Description would all be the same but, based on a dropdown of where the change needs to be (Server, Laptop, VDI etc) I would need to provide a different set of questions to the requestor for each, as well as create a different record for each option that is checked BUT keep the information from the first set of questions the same for all.

I hope this makes sense. I am self taught using websites and videos, so any help would be greatly appreciated.
