Form Error

I'm trying to fill out a form online , before the form loads it tells me to start a free trail. Once I hit start free trial I get an error message stating to contact you guys.

    @Anonymous If you clear all cache and cookies, close all tabs and windows of browser, open the browser, sign in again, go to make.powerapps.com - does the issue persist or is it resolved after that?
