Version history SP list item

One of my flows didn't function correctly (a few months ago and of course I cant take a look at it now).  Do you know of any reason that a list item would show so many versions at the same time when filling out a list item?  This was supposed to be going for approval and had some issues.   I usually don't see this many versions at the same time on items after they enter to submit them.  I'm seeing it happen with a certain person so I'm wondering if there is something they might be doing causing this that could solve this mystery?

If you click on the title column of a list item and edit the fields, a save takes place as you move from column to column.  If you click on "Edit all" after the form comes up, nothing is saved until you click on the save button.  The behavior that you have seen is normal if "edit all" is not clicked on.

I just ran a test where I did not click on edit all:

hmmm, this is happening after the initial creation without clicking on Edit or Edit Item.  It's a customized form with PowerApps not a Canvas App. I apologize I forgot to mention that before.

There are updates through my workflow, but those look ok. I just tested editing an entry using both Edit and Edit item and did not get the outcome you did.  Everything just had a single update and not multiple.  hmmmmm
