Error when trying to retrieve data from network

I am running into these two error when I shared my app with the team mates.  

All my SharePoint list are single line of text.

 

I had used a Feedback List which have all the country and company name

This is my app

for Country dropdown i had used

and for Company dropdown I had used 

B8_3-1677076431319.png

 

rest of the two are TextField.

 

I had Shared the Apps and sharePoint list Also but still.
