We would like to create a form identical to this one - https://www.cognitoforms.com/Oyerinsurance/PhoneAppointmentLogCopy . This form expands based on the questions answered. We also need it to be able to fill out in the background a Mississippi Insurance ID Card as well as an Binder for Auto Insurance. So we would like to enhance the form as we are filling out data to have create the appropriate forms for the auto and homeowner evidence of insurance as needed. Once the form is completed, it would then be emailed to our Client Processing Department for processing.

We are looking to hire someone to help us develop a workable form for our insurance agency. We do not have the skill to do this ourselves.

Do you want to expand the form based on the choices about Radio/ Checkbox?
Do all the DataCards on the page belongs to the same EditForm?
Do you want to submit all the information to the same table?
According to your description, PowerApps can meet your needs.
About expend the form based on the choices about Radio, you could set the form’s Visible: If(Radio2.Selected.Value="Yes",true,false)
About expend the form based on the choices about Checkbox, you could set the form’s Visible: If(Checkbox1.Value=true,true,false)

Do all the DataCards on the page belongs to the same EditForm? I'm not sure about the answer to this question. But based on my reading of DataCards the I think the answer is yes.

Do you want to submit all the information to the same table? If I understand this correctly the answer is yes.

Would you know of anyone who could create this for us?

1、expend forms by radio,checkbox
About expend the form based on the choices about Radio, you could set the form’s Visible: If(Radio2.Selected.Value="Yes",true,false)
About expend the form based on the choices about Checkbox, you could set the form’s Visible: If(Checkbox1.Value=true,true,false)
2、submit serval forms at the same time to the same record:
I recommend you use the function of Patch instead of SubmitForm.
Form1: to fill in the column of Title
Form2: to fill in the column of price
Foem3: to fill in the column of amount
Set the button’s OnSelect:
Patch(fruit,Defaults(fruit), {TitleataCardValue23.Text, price:Value(DataCardValue13.Text), amount:Value(DataCardValue3.Text)}
)

3、I recommand you use servel forms in one page.Choose the fields you need in every forms.
