Trying to build a flow based off an item being modified

Can someone please guide me on how to set up this flow? I want the email to be delayed until the date entered in the app, but I am not sure how to do that with the trigger being when an item is modified. What is the formula you use as the timestamp to delay an email based off an entry in a column from sharepoint? Any help would be greatly appreciated. Thank you in advance!

    There is a step for to pause the flow until a specific date/time, but the max is 30 days if i recall right.
