Data Source Schema for Power Apps for having fields on both rows and columns

I have a requirement where my form should take input as shown in below picture. Basically my fields are not just columns but also rows (Matrix). How should I plan my schema on SharePoint or SQL server. Single SharePoint list or multiple, single SQL table or multiple. Please advice how.
