Populating a Lookup Column with Existing Data

I have a table named "Patients" with 9000+ rows of data. Each row has a value in a column named "Insurance". I want to make this field a Lookup field so the user can choose the correct insurance when adding a new record but also show the existing "Insurance" if it is already populated. I have created a lookup column in the "Patients" table called "Insurance Lookup" which points to the table with all of the insurance names ("Insurance Table"). The "Insurance Lookup" field works fine on my form, but I need all of the existing "Insurance" data to populate in this field. How exactly can I achieve this?

    Hi @lpatterson7 ,

What's your data source? SharePoint or Dataverse?

    Best Regards,
