Question with Multiple Choice Options repeats two times

We have a bot the provides options to select 3 topics. Before the user responds the question is prompted second time. If the 'How many prompts' is set to 'Don't repeat' it goes to 'Action if no entity found'. Is there a way to stop this? Below is the screenshot.

It seems like the topic is getting triggered twice somehow. I attempted a similar approach to yours, but I have not encountered this issue. Please debug the chatbot to see how topic /question nodes are being triggered.

Question with Multiple Choice Options repeats two times

Could not find why this happens. Found more to this issue. After a simple welcome message, the first question of the session behaves this way. None of the further questions repeat. The first question when it appears for the first time does not wait for the user answer and repeats the second time immediately.
