Hi @GUOGUO :

Do you want to customize the SP Form in depth? (E.g. adding notes and hyperlinks)

If so,I suggest you try Custom Form:

https://docs.microsoft.com/en-us/powerapps/maker/canvas-apps/customize-list-form

Custom Form is based on Power Apps, so if you encounter any problems in subsequent development, you can also ask questions in the Power Apps forum.

Hi @GUOGUO ,

You can add a description to the field.

It doesn't seem to be possible to display a hyperlink.

There was a suggestion I read to create a multiple line of rich text field and style/position that as a description but not sure if it's worth the effort.

It works.
Though, the performance is not good . It is very slow when i create new item in the list.

Hi @GUOGUO :

This is because the custom form takes some time to load for the first time. After it is cached, adding records again will be much faster (without closing or refreshing the current page).
