New created could workflow always have "action" step setup disappeared after 1-2 days running

This issue already happens when I created could workflow,  including last Friday I created V3 email notification connect with SharePoint list update as a trigger, everything runs
fines but this morning I found action step just removed without any motion I did or any error message I receive. 
 
Anyone has any similar experience or knows the solution please help

    You now need to expand the Apply to each loops in order to view the actions:
