Hi i have added the Modified date into my form, but its shown as this format "MM/DD/YYYY Time PM"
i want it to be "DD/MM/YYYY Time PM"

If you use
Text(ThisItem.Modified, "dd/mm/yyyy hh:mm am/pm")
It should display as you require.

Check the link but I think it should be something like this:
Text(YourField; ShortDateTime)
