SharePoint list add date column to outlook calendar

Would someone please review my workflow. If a column with date changes in a SharePoint list, an item must be created in outlook calendar. If the date changes again, a new item must be created and the old one deleted. The flow worked well but over time the flow created duplicate items and stopped working properly. What am I doing wrong!!

Dear P,
As your flow was working previously, it seems unlikely that the issue lies there.
How the duplicate items created? Was this done in 1 flow run ... or did you see that the same sharepoint event triggered multiple flows at the same time? This duplicate triggering from sharepoint seems to be a common issue (looking at the forum entries pointing towards the same).

If it is a duplicate Sharepoint trigger (and you cannot live with it), I suggest you try the following:
Set the flow concurrency to 1 to ensure that only 1 flow run can run at any time (the others if triggered get queued). In addition to this, you build in a check in your flow to check that if you want to create an item, you check first if this item does not yet already exists. If it does exists, don't create another. This check should prevent duplicate item creation.

Thanks for the explanation Koen, 1 current is running and that is updating item in outlook. I have tried several steps, it works for a while, but then the flow creates a new item in outlook every 20 seconds.
