Updating a Record Created By The User/Patch Command

I'm having problems figuring out the Patch command for a specific use.

Here's my setup:

Screen 1: User enters name, ID #, has submit button using the SumitForm(MyFormName) to create the record in my Sharepoint list.

Screen 2: User will read some information and there's a toggle and a submit button. I've added the toggle from the Forms>Edit menu and customized it to only show the one field (column) that I want to add data for but I'm running into issues with the submit button. I think I'd use this format: Patch( DataSource, BaseRecord, ChangeRecord1 [, ChangeRecord2, … ])

For base record, how do I say: I want you to go find the form that this user submitted on step 1?

For change record, how do I say: Look at the toggle I put on the screen, take the value of that toggle, and update my Yes/No column on the Sharepoint list for that record?

Is the toggle part of an edit form that can be submitted with SubmitForm()? If so instead of the Patch function you can use set the Item property of that form using the MyFormName.LastSubmit property. So the Item Property would look like this:

Lookup(DataSource, ID = MyFormName.LastSubmit.ID)

Then you can SubmitForm() and it will patch to the correct record.
