please check to make sure you are using Outcome in the condition.
if you use Responses it will not work correctly,
basically Responses = an array of all outcomes
Outcome = The outcome from all the responses

Please try to use the following method to get comments and you need to remove Apply to each and Apply to each 2.
Besides, please try to use Outcome to check the approval result, you could refer to the following thread to get the different configuration of Condition.
