Hello,

I have been tasked with developing a method to embed a form into a Sharepoint Site, and the form must have the ability to do the following:

- Automated filling in name of person signed into the Sharepoint Site

- Each person has certain Companies associated with them on a Sharepoint List. The companies associated with the person autopopulate into a dropdown list in which the user can chose which company to review using the form.

- The user is then prompted with some hierarchy questions (ie for Question 1, have yes/no for answers and if no is chosen, a bunch of options come up to chose from). Each of these options need to have weighted scoring (ie if option 1 is chosen, 40 point will be added to a score. If option 2 is chosen, 20 points will be added to a score). At the end, the final total score will be summed to determine a final letter grade.

Is this possible? I do not necessarily need the entire process on how to do it laid out bc I do not need to actually create this form, I just need to know if this is possible and the general ideas/concepts needed to produce this.
