Percentage Bar, based on Star Rating Value

Hi, 

 

I have a form in my App which pulls from a number column in a SP List. 

 

As you can see in the image below, this item has been rated 5 stars. What I'd like to achieve is as shown underneath the stars, a bar representation, so the grey is the background, the green should fill the grey to represent 5. The rating is set to min 0 max 5.

Any help with the code I should apply would be great, thanks.
