Help Building a Time & Task Tracker App

I'm trying to build a simple app that can fill in a very simple excel table in some way. I want to allow field technicians to record when, where and what they did in 1 hour increments as a sort of time audit. Ideally, I'd like it to save a new file with the name of the technician, date and jobsite in the file name and have it save to a SharePoint. The goal is to make it very quick easy for field techs to use. I have the basic design, but not the knowledge to make it function.
