How can I identify which 'list form' was used to submit list item?

I am building a list to house all requests for our Learning and Development Team. These requests will come through 4 'list forms' that will be embedded in a button on our team sharepoint page, each with a different type of request e.g. individual training, group training, consultation, and product development.

I would like the list to have a column that is named 'request type' and shows which 'list form' has been submitted rather than having a user click the form button on sharepoint (selecting the form for their request type) and then having to select the request type in a question on that form again.

Is there any way of identifying which 'list form' has been submitted so I can avoid this duplication of effort and improve user experience?

Or is there a way I can at minimum have the question in each form but make the answer default to match that form? (at the moment if I select a default for the list column it will be the default in all the 4 forms).

I am finding it frustrating the variations between 'forms' and 'list forms' but have to use 'list forms' in order to have active directory lookup when searching for a learner name and approver name (and I am not skilled enough to build something better in power apps yet).

Any help would be greatly appreciated.

    If i am right > these list forms will populate the form submissions into its own list right ? These list datas finally needs to be added to another final list - correct ?

    Hi @CU24120541-0 - I don't think the native List Forms have the out-of-the-box ability you're looking for.  Currently, I believe you may have three (3) workaround options for this functionality:
     
    1. Quick + Dirty = Have each form populate different fields in your target list, then identify the source form accordingly (either manually or via flow)
    2. Clean but More Layers = Use @Nived_Nambiar's suggestion and have each form populate its own separate list, then use power automate to merge the contents of each list into a single target list (identifying each source accordingly)
    3. Beautiful but Complicated = Forgo the native forms and build your own custom forms via PowerApps to self-identify which form was used (credit to @RobElliott for his suggestion here)

    Since you indicated that you're not a strong Power Apps user, I recommend option 2.  Though it requires a bit of setup time to create each of the prerequisite lists and merging flows, maintenance and support will be MUCH simpler than either of the other options.
