Approval Form in Teams Controlled by Flow

I would like to have a form (or several if one cannot do it) in a MS Teams channel that can be filled out by an approver to confirm that a point in the development process is approved. Via flow I would imagine this could then send an email to me and the approver as a confirmation.

The challenge I am having is how to have the form be smart enough to know (1) at which point in the process it is approving, and (2) which project this approval is for. I could simply have fields in the form for the approver to manually enter the process point and the name of the project they are approving, but this is error prone so I am wondering if there is a more elegant solution? Perhaps via Power Apps integration? The setup I have at the moment is one project per team channel.
