Flow Randomly Fails: Using 2 different FORMS to UPDATE the SAME SP Record

We have a multistep application process where applicants will submit their initial application by way of a MS Form, if it is approved (approval process) then an email is sent to the applicant with their unique applicant ID# and a link to another MS Form.  I set up the flow from another post I found (click here) and it's been working great.  Recently however, I've been getting a number of submissions that are failing at the UPDATE RECORD part of the flow and I can't figure out why as some fail and some do not:
