I was wondering if an App can be developed in Canvas mode and embedded into a SP list/library form page (not a web page).  Has anyone tried this approach?  I really would like to know before I spend too much time experimenting.

    Yes, you can do this.  You need to set the "Enable app embedding user experience" on in the app settings.
