'Create Item' creating duplicate entries/items in SharePoint List

My canvas App uses a SharePoint List as the database, to which I am creating a record and appending an attachment based on user inputs. The attachment (a text file) is also created as part of the flow after the item is created [Create item -> Add attachment]. 

 

This flow worked well ~1 month ago when I last worked on the app, but now when the flow is triggered it creates 2 new items - one with the attachment - and another with no attachment.  

 

Has anyone else run into this before? Is there a known issue or am I missing something?

    Hi @JSCS_2022 

     

    If you haven't already, please could you check if there are any other flows running that would create a record in question thereby resulting in a duplicate?

    Alternately, please turn off the flow you're talking about and see if the record still getting created when the trigger condition is met. 

    If no duplicate flow (or record creation action in any other flow) found then please share the flow steps with the community for review.
